VITAL STATISTICS OFFICE Birth & Death Records
THE TEXARKANA TEXAS CITY HALL - VITAL STATISTICS OFFICE IS CURRENTLY CLOSED TO WALK-IN CUSTOMERS. DROP-OFFS AND MAIL-INS ARE ACCEPTED.
To obtain a birth or death certificate, customers can mail-in or drop-off an application for a long form certificate at the Texarkana Texas City Hall, Monday-Friday 8:00am - 5:00pm at 220 Texas Blvd Texarkana, Texas 75501. Applications are located at the front reception desk with a drop-off box located behind the front reception desk.

General Information
- In order for staff to process request, completely fill out the application to sufficiently identify the record, provide an acceptable ID, sign application in front of a notary for verification, and include the appropriate fees required.
- Due to changing state laws, a county or city issued birth certificate may not be accepted in some states. If your state of residence requires a state issued birth certificate, you can order from the State of Texas at www.texas.gov/texas-vital-records/
Mail-In and Drop-off Applications (City Hall)
- Please allow 4-6 weeks for processing and mailing
- Mail-in and drop-off applications must be notarized
- All certificates will be returned via mail
- Make checks payable to: City of Texarkana, Texas
Birth Certificates: Application: English (PDF) | Spanish (PDF)
- Texarkana, Texas, Wake Village or Nash birth certificate holders will receive the long form which is the most widely accepted record. This record will also display corrections that have been made to the birth certificate. The Passport Office and some cruise lines require the long form.
- All other Texas birth certificate holders can obtain the abstract form which will work for everything except leaving the United States. To obtain the long form, you can contact either the city or county where you were born, or the State of Texas at www.texas.gov/texas-vital-records/
- To obtain a Native American Roll Number, you must order your certificate from the State of Texas at https://www.dshs.texas.gov/vs/
- Certified Birth Certificate, long or short form $23 +$2 mailing fee.
Death Certificates: Application: English (PDF) | Spanish (PDF)
- Our office only processes Texarkana, Texas, Wake Village, or Nash death certificates
- For all other Texas death certificates, contact the city or county where the decedent died, or the State of Texas at www.texas.gov/texas-vital-records/
- Certified Death Certificate: $21 +$2 mailing fee.
NOTE: Each additional Death Certificate ordered of the same record, at the same time, is $4 per copy.

- Order all Texas birth and death records online at hwww.texas.gov/texas-vital-records/
- Online instructions include the time frame and prices for regular and expedited services.
Other ways to obtain birth records:
- (Short Form) Bowie County Clerk’s Office from Monday-Friday 8:00am - 4:00pm (Closed for lunch 12:00 pm-1:00 pm) at 710 James Bowie Dr. New Boston, Texas. Call (903) 628-6740 for any additional questions. Can usually be obtained same day.
- (Long Form) Order online direct from the State Vital Statistics Office at www.texas.gov/texas-vital-records/. You can choose to expedite. Or call (888) 963.7111.
Changes to Birth or Death Certificates
- ALL changes are made by Texas Dept. of State Health Services by using the following forms: VS-170 Correcting a Birth Certificate or VS-172 Correcting a Death Certificate.
- These forms will include full instructions, a list of required documents and fees associated with the request.
- Changes could include: spelling or date errors, legal name changes, adding, removing, or replacing a parent, correcting the place or time of event, or correcting the sex, race, or age of a certificate holder or parent.
